Your appointment can be confirmed by text, e-mail, or phone, whichever you prefer. Please return any missed phone calls from our offices and be sure to save our numbers to your contacts.
New patients are asked to arrive 15 minutes before their scheduled appointment time. This allows ample time to fill out necessary paperwork and enter you into our systems. Current patients can arrive 5-10 minutes prior to scheduled appointment times.
Every patient will need to bring a state issued ID, insurance card, list of medications, and a method of payment to each appointment. Minors must be accompanied by a parent or guardian.
If you can’t make your appointment, please let us know at least 24-hours prior to your scheduled appointment time in order to avoid a cancellation fee.
